Rather than print any forms out in advance, I advice that you bookmark the link and print the most up-to-date version when you need it (using the current form can be very important).
This is the law in Ontario that regulates landlords and tenants. I think you will find it has reasonably easy-to-understand wording. I often use the “find” function on my browser to find specific sections of the act.
Rent Check Credit Bureau

If you join their service, they have many forms available in addition to supplying credit reports and other tenancy reports for Landlords.
